Texas Consumer Notice (Texas Health & Safety Code 181.105, effective September 1, 2025).

This page explains how to request health care records, where to contact the Texas licensing authority, and how to file a consumer complaint.

1) Request your health care records

Choose one of the options below to request records.

Include the following

  • Your full legal name at the time of services
  • The date range of records requested (mental health records are required to be kept for 7 years after the date of last contact)
  • Type of information requested (treatment notes, dates of service, billing records)
  • How you want records delivered (for example: secure email, portal, mail)
  • If you want records released to another person or provider, complete a signed Consent to Release or Obtain Information (PDF)

2) Contact the Texas Behavioral Health Executive Council

Texas LPCs are regulated through the Texas State Board of Examiners of Professional Counselors, which is part of the Texas Behavioral Health Executive Council (BHEC).

3) File a consumer complaint with the Texas Attorney General

Consumers may file a complaint with the Texas Office of the Attorney General, Consumer Protection.
